Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through in-depth interviews, observations, and careful review of medical records to identify emotional, social, and environmental factors impacting patient recovery.

• Collaborate with physical and occupational therapists, physicians, and rehabilitation specialists to design and implement individualized treatment plans that reflect the psychosocial needs of each patient.

• Facilitate access to essential community resources, such as support groups and vocational rehabilitation services, to enhance overall patient support systems.

• Educate and empower patients and their families regarding available post-rehabilitation services to ensure a smooth transition to independent living.

• Play an active role in discharge planning by coordinating with healthcare providers to facilitate a safe and effective transition to home or other care settings.
