Responsible for the evaluation, clinical assessment, treatment plan, performance of therapeutic procedures, and progression of physical therapy patients. Responsibilities also include consultation with medical staff and health professionals, completion of clinical records and insurance forms, and supervision of assistive personnel. MINIMUM QUALIFICATIONS : EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Current physical therapist licensure in the applicable state where services will be performed:
MD: Physical Therapist Licensethrough Maryland Board of Physical Therapy Examiners OR Physical Therapist Compact License
OH: Physical TherapistLicensethrough Ohio Occupational Therapy, Physical Therapy, and Athletic Training Board OR Physical Therapist Compact License
PA: Physical Therapist License through Pennsylvania State Board of Physical Therapy OR Physical Therapist Compact License
WV: Physical Therapist License through West Virginia Board of Physical Therapists OR Physical Therapist Compact License
2. Valid driver's license.
3. CPR certification within 30 days of hire date.
PREFERRED QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Eligible for membership with APTA or WVPTA.
EXPERIENCE:
1. Experience as a Physical Therapist
CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. Performs evaluation procedures according to p/p or standard of care including but not limited to musculoskeletal/ortho test, neuro/motor test, gait analysis, manual muscle test, functional analysis, prosthetic/orthotic evaluation, and Oasis worksheet. 2. Effectively deliver and analyze physical therapy treatment, including but not limited to modalities, therapeutic exercise, and manual skills as per policy and procedure. 3. Implements home programs; meets patient/family educational needs and assesses patient understanding as per policy and procedure. 4. Supervises assistive personnel and physical therapy students affiliated with accredited physical therapy programs per standards. 5. Provides care, based on physical, psychosocial, communication, safety, and education level and related criteria for the infant, pediatric, adolescent, adult, geriatric patient per the established age-specific education/standard. (Separate Competency will be completed prior to caring for infants, pediatrics, or adolescents.) 6. Communicates patient status/progress with home care team, both informally and at staff meetings. 7. Documents supervisory visits as per standard. 8. Paperwork completed and submitted in a timely manner. 9. Monitors and facilitates staff compliance with maintaining a safe and secure environment. 10. Performs CPR when directed by patient's appropriate representatives. 11. Demonstrates bag technique per policy and procedure. 12. Apply and maintain equipment according to the Policy and Procedure manual. 13. Familiar with supply requisition systems as per policy and procedure. 14. Perform treatments/procedures according to specific standards. 15. Performs and monitors patient teaching according to the policy and procedures. 16. Coordinates the care team in order to facilitate a change in the level of care in compliance with the change in level of care standard. 17. Identifies the stages of death and dying. PHYSICAL REQUIREMENTS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling are necessary body movements utilized in performing duties throughout the work shift. 2. Must be able to lift, move, and position patients of all weights, with varying amounts of assistance in a fair manner on a regular and recurring basis. 3. Must be able to communicate effectively. 4. Visual acuity must be within normal range.
5. Must be able to perform heavy work; exerting in excess of 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects. WORKING ENVIRONMENT: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Exposure to bodily fluids, human tissues, contagious diseases, sharps, and explosive gases. 2. Exposure to toxic gases, fumes, and odors. 3. Exposure to high stress and constant interruptions. 4. The majority of work is performed in the patient's home and may require extensive travel. SKILLS AND ABILITIES: 1. Must have strong communication skills, written and verbal. 2. Must be able to delegate tasks to PTAs. 3. Must demonstrate time management skills. 4. Must be able to problem solve in an effective manner. 5. Must show an interest in physical therapy continuing education. 6. Ability to communicate effectively both verbal and written. 7. Demonstrates knowledge of and compliance with safety policies and procedures. 8. Demonstrates knowledge of compliance of Federal, State, and Local regulations/laws.
