Job Description:

Home Health Physical Therapist Summary:Receive and review physician therapy orders for Home Health and Hospice patients. Complete comprehensive patient evaluations and establish written treatment plans with short-term and long-term goals. Provide skilled physical therapy interventions, including therapeutic exercises, stretching, mobility training, balance activities, pain management techniques, and functional rehabilitation. Assess home safety and recommend equipment, assistive devices, or environmental modifications to promote independence and prevent falls. Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes. Educate patients, caregivers, and family members regarding treatment plans, home exercise programs, safety measures, and expected outcomes. Maintain accurate and timely clinical documentation, including evaluations, progress notes, discharge summaries, recertifications, resumptions of care, and discharge documentation. Collaborate and communicate effectively with physicians, nurses, therapists, and other healthcare professionals involved in patient care. Travel to patient homes and other care settings to provide therapy services.Must have current PT licensure in the state of MontanaMust have valid driver's licenseBLS Certified
