RN Hospice- Per diem

Location: Sacramento or surrounding areas 10–20 hours per week, including rotating holidays, on-call shifts, and weekends. On-Call Hours: 6:00 PM – 9:00 AM Key Responsibilities Complete patient assessments & update care plans Provide symptom & pain management Educate patients/families on care and medications Coordinate with physicians and interdisciplinary team Document timely, accurate clinical notes Manage assigned caseload; ensure safe care environment Participate in on‑call rotation Requirements Must have: Current RN license CPR certification Valid driver’s license & insured vehicle 1+ recent year of RN experience or BSN Ability to lift/position patients; frequent standing/walking Preferred Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
