Main St., Decatur, IL 62526Schedule: Varied hours between 6a-4:30p Monday – Saturday; based on MAR program and clinic needsFLSA Status: Non-ExemptPosition Type: Full-TimeWork Setting: In-PersonPosition Overview:In keeping with the Mission, Vision, and Corporate Values of Heritage Behavioral Health Center, the LPN – Medication Assisted Recovery joins the Heritage nursing team to provide support and medication services to assist adults with a diagnosis of substance use disorder who are enrolled in the agency's Medication Assisted Recovery (MAR) program. Services are provided at Heritage's downtown office in Decatur, Illinois.This position reports to the RN Program Manager – Substance Use and works collaboratively with clients, the nursing team, physicians, providers, and other members of Heritage teams to support each individual's recovery.This position involves regular direct contact with individuals served by Heritage, including adults living with substance use and mental health conditions. A compassionate, professional, trauma-informed approach is essential.Leadership & Team Expectations:Represent Heritage's mission and values through professional, respectful conduct at all timesApproach all interactions with clients using trauma-informed, recovery-oriented practices that are sensitive, empathetic, and effectiveWork collaboratively with clients, the nursing team, physicians, providers, and other Heritage teamsCommunicate openly and proactively with the RN Program Manager regarding client needs, concerns, and program operationsMaintain client confidentiality in accordance with HIPAA and 42 CFR Part 2Demonstrate flexibility and a team-oriented mindset in the midst of ambiguity, uncertainty, and changeFollow all Heritage policies, procedures, and safety standardsCore Responsibilities:Medication Administration & Nursing CareProvide daily medication administration in the Medication Assisted Recovery programProvide nursing assessments, medication administration, medication monitoring, and medication training to assist clients with learning how to self-administer medications, monitor response to medications on target symptoms, properly secure and store medications, and communicate concerns to providers; support referral and care coordinationAccurately document in the electronic health record daily on interventions, the client's response, and the client's progress in treatmentCare Coordination & Client EducationCoordinate holistic care for individuals receiving medication assisted recovery, including primary care, labs, mental health or substance use treatment, and other identified needs/careProvide education on health and wellness, including, but not limited to, chronic health disease self-management, infectious diseases, and primary careTeam Collaboration & ComplianceParticipate in a team-based approach to providing treatment, including collaboration with clinicians, nursing, healthcare providers, and other partners invested in the individual's recoveryAttend clinical staffing(s) to collaborate with providers to coordinate treatment, including counselors, engagement staff, nursing, physician(s), and clinical leadersAdhere to agency policies and procedures, accrediting body requirements for the Opioid Treatment Program (CARF), and federal and state guidelines (Illinois Division of Substance Use Prevention and Recovery, DEA, and SAMHSA)Engage in ongoing learning of current best practices in behavioral health treatmentPerform other duties as assigned consistent with this rolePerformance Expectations / What Success Looks Like:Clients receive safe, accurate, and timely medication administration and monitoringDocumentation in the electronic health record is complete, accurate, and entered dailyClients feel supported, respected, and treated with dignity throughout their recoveryCare is well-coordinated across primary care, labs, and behavioral health servicesThe nursing team, providers, and program leadership view this position as a reliable, collaborative teammateThe program maintains compliance with CARF, DEA, SAMHSA, and state requirementsKnowledge, Skills and Abilities:Familiarity with addiction/recovery and behavioral healthThorough understanding of trauma-informed care practices, essential for providing optimal support and services to individuals in a manner that is sensitive, empathetic, and effectiveAbility to work as a team playerAbility to tolerate ambiguity, uncertainty, and changeAbility to navigate electronic health recordsDemonstrated above-average computer skills; proficient in Microsoft Word and ExcelStrong attention to detail and accuracy in medication handling and documentationAbility to maintain client confidentiality in accordance with HIPAA and 42 CFR Part 2Education and Experience:Required:Minimum 21 years of ageAn active, undisciplined State of Illinois LPN licensureA valid driver's license, reliable means of transportation, and proof of automobile insurance coverageAbility to obtain and maintain current CPR certificationPreferred:Experience in substance use treatment, behavioral health, or medication assisted recovery settingsExperience with medication administration in an outpatient or Opioid Treatment Program settingNote: Equivalent combination of education and experience will be considered.Physical Requirements and Working Conditions:Ability to stand, walk, and remain on feet for extended periods during medication administrationAbility to sit, stand, and work at a computer for periods of timeAbility to respond calmly and effectively in crisis or emergency situationsRegular direct contact with individuals served by Heritage, including those experiencing substance use and mental health challengesMay require travel between Heritage facilities and community locationsMust adhere to all Heritage policies, procedures, and safety standards
