Social Worker II

Role: The Social Worker facilitates patient care by coordinating resources, conducting psycho-social assessments, and developing transition plans to promote safe discharges and prevent readmissions. Responsibilities: Assess patients' transition needs, assist with complex psycho-social interventions, and manage cases involving adoption, abuse, or neglect. Lead multidisciplinary care conferences, communicate care plans with patients and families, and ensure accurate documentation. Oversee and implement transition plans, ensuring compliance with regulatory standards and patient satisfaction. Qualifications & Skills: Must hold a Master's of Social Work (MSW) and be licensed or certified as required. Preferred experience includes two years in an acute hospital setting and certification as an Accredited Case Manager (ACM). Strong assessment, communication, care coordination, and documentation skills are essential.
