Key Responsibilities\n- \n
- Conduct thorough psychosocial assessments to identify the needs and challenges of patients and their families affected by chronic or life-threatening illnesses. \n
- Collaboratively develop and implement individualized Plans of Care (POC) with the healthcare team, patients, and their families, aimed at addressing specific diagnosis-related needs. \n
- Continuously evaluate and modify existing treatment plans based on the evolving needs of patients and families, ensuring that care remains relevant and effective. \n
- Participate actively in team meetings to discuss patient progress, share insights, and work collectively towards meeting departmental goals and objectives. \n
- Demonstrate professional competence in patient and family care during field supervision, consistently applying best practices in therapy. \n
Your expertise will focus on providing support to patients and families dealing with chronic or terminal illnesses, including those in hospice care. You will be equipped to handle a variety of emotional and social challenges, ranging from grief and loss to daily coping strategies. By incorporating expressive arts therapy, you will help patients discover new avenues for expression and healing.
Qualifications\n- \n
- A current and valid License as a Licensed Clinical Social Worker (LCSW) in California is a must. \n
- A Master’s degree in Social Work (MSW) with a minimum of two years of post-licensure experience is required. \n
- Possess comprehensive knowledge of counseling techniques, crisis intervention strategies, and the ability to make independent and informed decisions related to direct service. \n

- Experience in facilitating group therapy sessions and planning therapeutic activities is highly desired. \n
- A background in working with hospice or palliative care settings may be advantageous. \n
