Work Environment

This position is fully onsite, focusing on home care, which allows for a personalized treatment environment tailored to each patient's needs. You will interact with patients within their homes, providing therapy sessions that are comfortable and conducive to rehabilitation. Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians, assessing their physical abilities, limitations, and personal goals.
• Design and implement individualized treatment programs that align with each patient's health status and therapeutic objectives.
• Utilize a variety of techniques including manual therapy, therapeutic exercise, and modalities to facilitate rehabilitation.
• Monitor and document patient progress meticulously, making adjustments to treatment plans as necessary based on patient response and recovery.
• Provide education to patients and their caregivers on rehabilitation processes, prevention strategies, and home exercises.
• Collaborate with a multidisciplinary team to coordinate care and maximize patient outcomes using evidence-based practices.
• Ensure that the highest levels of customer service are maintained, enhancing patient satisfaction and trust in our services.
• Meet utilization goals and contribute to departmental objectives to ensure the success of our home care program.

Patient Population / Client Focus

Your caseload will encompass a diverse range of patients, including those in orthopedic rehabilitation, pediatric therapy, neurology, and sports medicine. This variety provides an enriching work experience, allowing you to apply different therapeutic methodologies suited to each patient's condition. You'll play a key role in re-establishing movement and functionality and fostering independence among individuals who are often navigating significant physical challenges.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.
• A valid state license to practice physical therapy in Florida.
• Certification in Basic Life Support (BLS) from an approved organization.
• Strong communication skills, both verbal and written, with a focus on effectively educating patients and their families.
• Proficiency in using electronic health records and basic computer applications necessary for documentation and communication.
• A genuine passion for building strong relationships with patients, enabling them to exceed their functional goals.

Preferred Experience

• Experience in home health care or outpatient therapy settings is highly preferred.
• Proven ability to work independently and manage a caseload effectively.

Compensation and Benefits

This position offers a competitive annual salary ranging from USD 80,000 to 100,000, dependent on experience and qualifications. In addition, you will be eligible for performance-based incentives that celebrate outstanding contributions to patient care.
