Job Title: Certified Physical Therapist (Home Health) Job Location: Oakland, MI Job Type: Contract Job Description :Conduct patient evaluations and reassessment sDevelop and implement individualized treatment plan sProvide therapeutic exercises, gait training, balance training, and mobility trainin gEducate patients and caregivers on home exercise programs and safet yComplete timely documentation and progress report sCollaborate with physicians and healthcare team member sEnsure compliance with Medicare and agency guideline sRequirement : Graduate of an accredited Physical Therapy program (DPT or equivalen t)Current Physical Therapist licen seCurrent CPR certificati onMinimum 1 year of Physical Therapy experience (Home Health preferre d)Valid driver's license and reliable transportati onStrong communication and documentation skil lsAbility to work independent ly
