Organization VHCS Clinical - Nursing Job Summary Works in collaboration with the RN or a licensed healthcare team member to provide direct care to patients, both pediatric and adult in the home setting. Shift M-F (8:00 a.m. - 4:30 p.m.) Department Summary Vanderbilt Home Care Services – Intermittent Care provides short‑term, medically necessary services to patients in their homes under physician direction. The division focuses on supporting individuals recovering from illness, surgery, or acute exacerbations of chronic conditions, allowing them to receive skilled care while maintaining independence in a familiar environment. Intermittent services are delivered on a part‑time or as‑needed basis and include skilled nursing, physical therapy, occupational therapy, speech‑language pathology, and home health aide support. Care is tailored to each patient’s plan, with an emphasis on restoring function, improving mobility, managing symptoms, and promoting self‑care. The program primarily serves patients transitioning from hospital or post‑acute settings, making it a critical component of the care continuum. Clinicians collaborate closely with physicians, case managers, and caregivers to ensure coordinated, seamless care. Frequent assessments and plan adjustments are conducted to align with patient progress and evolving needs. The Intermittent Home Care team prioritizes quality outcomes, patient safety, and reduced hospital readmissions through evidence‑based practices and proactive care management. Advanced technology such as electronic health records and remote monitoring tools support timely communication and enhance care coordination. Key Responsibilities Collaborates with patients/families and health care providers in the delivery of health care services. Partners with the healthcare team by following established plan of care to ensure patient progress and effective outcomes. Supports plans to promote health and a safe, healthy environment. Navigates intense medical scenarios with autonomy, under the direction of the supervising RN while maintaining compliance with the physician’s ordered plan of care. Administers intravenous fluids and performs sterile procedures. Recognizes and responds to clinical situations appropriately and timely to prevent patient decompensation and injury. Identifies and assesses patient needs and clinical changes, accurately reporting findings to the supervising RN and physician. Technical Capabilities Clinical Competency (Intermediate): Demonstrates mastery of nursing interventions such as wound care, point‑of‑care testing, vital signs, etc., and can independently deliver vital sign services. LPN Patient Education (Intermediate): Provides patient education independently and can train peers. LPN Care Planning (Intermediate): Participates in developing the plan of care, forecasts resources, sets priorities, and manages multiple assignments. Clinical Patient Documentation (Intermediate): Manages patient documentation, collects and records objective and subjective data accurately and timely. Position Qualifications Certification: Licensed Practical Nurse (LPN) – Tennessee. Experience: 1 year of related work experience. Education: Graduate of an approved discipline‑specific program.
