Services are provided in home health settings and focus on helping patients achieve greater independence, safety, and quality of life.ResponsibilitiesEvaluate speech, language, communication, cognitive, voice, and swallowing disordersDevelop individualized treatment plans based on physician orders and patient needsProvide evidence-based therapeutic interventions to improve communication and swallowing functionAssess and treat patients experiencing speech, language, cognitive, and dysphagia-related challenges resulting from illness, injury, or neurological conditionsEducate patients, family members, and caregivers on treatment strategies and home exercise programsRecommend communication aids, adaptive devices, and compensatory strategies as appropriateMonitor patient progress and modify treatment plans as needed to achieve therapeutic goalsCollaborate with physicians, nurses, caregivers, and other healthcare professionals to coordinate careCommunicate changes in patient condition and treatment response to the physician and Director of NursingComplete accurate clinical documentation, progress reports, and discharge summaries in a timely mannerTreat clients, families, caregivers, and team members with professionalism, courtesy, and respectQualificationsMaster 's Degree in Speech-Language Pathology from an accredited programCurrent Speech-Language Pathologist license in the state where services are providedCertificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred or required based on state regulationsMinimum of two (2) years of experience as a Speech-Language PathologistHome health or community-based experience preferredExperience evaluating and treating patients with communication, cognitive, and swallowing disordersStrong clinical assessment, communication, and documentation skillsCommitment to maintaining HIPAA compliance and patient confidentialityJoin Our TeamIf you're passionate about helping patients improve communication, cognitive function, and quality of life while enjoying the flexibility to create your own schedule, we'd love to hear from you.
