Rehab Without Walls® is seeking an experienced LCSW to join our Home & Community Program in Seattle. Responsibilities Perform comprehensive evaluations, assessing the social, emotional and support needs of patients and families Set measurable objectives that are formulated in conjunction with the rehab team Assist the physician and other health team members in understanding significant social and emotional factors related to the patient's health problems Instruct/counsel patients and families in coping with social and emotional responses connected with illnesses Participate in care coordination activities and act as a resource to other health team members in the identification and resolution of patient needs Identify and assist patients/families to utilize appropriate community resources to achieve measurable objectives Qualifications Master's Degree in Clinical Social Work (LCSW) Current Licensed Clinical Social Worker Licensure in the state of practice Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis CPR certification required Ability to lift 50 pounds Ability to move, lift, or transfer patients involving up to 100 pounds following safety procedures Requires fine motor skills, visual acuity, and walking/standing for extended periods Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently About Our Line Of Business Rehab Without Walls, an affiliate of BrightSpring Health Services, is a revolutionary neurorehabilitation program that moves individuals outside the walls of institutional settings into their natural home and community environment.
