About the Role
At Avir at Clarksville, you'll be responsible for:
- Providing direct patient care, including assessing patient needs, developing individualized care plans, and implementing interventions to promote optimal health and well-being
- Collaborating with interdisciplinary teams to coordinate patient care and ensure seamless transitions between departments
- Educating patients and families on disease prevention, treatment, and management strategies
- Maintaining accurate and up-to-date patient records, adhering to HIPAA guidelines and regulatory requirements
Responsibilities
- Provide compassionate and empathetic care to patients, families, and caregivers
- Collaborate with physicians, therapists, and other healthcare professionals to develop and implement comprehensive care plans
- Conduct thorough patient assessments to identify needs, goals, and limitations
- Implement evidence-based interventions to promote optimal patient outcomes
- Maintain accurate and timely documentation of patient care and treatment
