Position Summary

The Nurse Supervisor is responsible for supervising and coordinating the delivery of skilled nursing services in accordance with Colorado Department of Public Health and Environment (CDPHE) regulations, Medicare Conditions of Participation (42 CFR Part 484), Medicaid requirements, agency policies, and accepted standards of nursing practice. The Nurse Supervisor ensures the provision of high-quality patient care, clinical compliance, staff education, and quality improvement activities while promoting positive patient outcomes.

Essential Duties and Responsibilities

Clinical Supervision

• Supervise Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Home Health Aides (HHAs), and other clinical staff.
• Ensure patient care is delivered safely, effectively, and in accordance with physician orders and individualized plans of care.
• Review and approve nursing documentation for accuracy, completeness, and regulatory compliance.
• Monitor patient outcomes and implement corrective actions when necessary.
• Participate in case conferences and interdisciplinary care planning.
• Assist with complex patient assessments and clinical decision-making.

Regulatory Compliance

• Ensure compliance with:
  • Colorado Department of Public Health and Environment (CDPHE) regulations.
  • Medicare Conditions of Participation (42 CFR Part 484).
  • HIPAA requirements.
  • OSHA safety standards.
  • Infection prevention and control guidelines.
• Participate in agency surveys, audits, and accreditation activities.
• Maintain knowledge of state and federal regulations affecting home health services.

Quality Assurance & Performance Improvement (QAPI)

• Participate in Quality Assessment and Performance Improvement (QAPI) activities.
• Conduct routine chart audits.
• Monitor clinical quality indicators.
• Identify trends and recommend corrective actions.
• Assist with performance improvement initiatives.

Staff Supervision

• Orient and mentor newly hired nursing staff.
• Conduct competency evaluations and annual performance reviews.
• Provide ongoing clinical education and in-service training.
• Monitor staff productivity and compliance with agency expectations.
• Address performance concerns and recommend disciplinary action when appropriate.

Patient Care Coordination

• Coordinate services with physicians, therapists, social workers, and other healthcare providers.
• Ensure timely communication of changes in patient condition.
• Review medication management.
• Physician orders.
• Assist with admissions, recertifications, resumptions of care, transfers, and discharges as needed.
• Coordinate coverage for any patient and assume any if necessary. 

Documentation

• Ensure documentation complies with Medicare billing requirements.
• Review OASIS assessments for accuracy.
• Verify physician orders are signed in a timely manner.
• Ensure documentation supports medical necessity.
• Monitor the timely completion of clinical records.
• Responsible for the whole QA of the agency.

Risk Management

• Investigate patient complaints, incidents, and adverse events.
• Participate in root cause analyses.
• Implement corrective action plans.
• Promote patient safety initiatives.

Qualifications

Education

• Graduate of an accredited School of Nursing.

Licensure

• Current unrestricted Registered Nurse (RN) license in Colorado.

Experience

• At least one (1) year of supervisory or management experience preferred.
• Home Health experience strongly preferred.
• OASIS experience preferred.

Knowledge, Skills, and Abilities

• Thorough knowledge of Medicare Home Health Conditions of Participation.
• Knowledge of Colorado home health regulations.
• Strong clinical assessment skills.
• Leadership and supervisory abilities.
• Excellent communication and interpersonal skills.
• Strong organizational and time management skills.
• Ability to prioritize multiple responsibilities.
• Computer proficiency, including Electronic Medical Records (EMR) systems and Microsoft Office.

Physical Requirements

• Ability to travel to patient homes throughout the assigned service area.
• Valid Colorado driver's license.
• Reliable transportation.
• Ability to lift up to 45 pounds.
• Ability to stand, walk, bend, and assist patients as needed.

Working Conditions

• Office setting and patient homes.
• Exposure to infectious diseases and environmental hazards common in healthcare.
• Occasional evening or weekend responsibilities.
• Participation in on-call rotation, if required.
