Job Summary Licensed Practical Nurse (LPN) at Westminster Canterbury Richmond provides nursing care for residents of the Mary Morton Parsons Health Center. The LPN performs assessments, care planning, implements interventions, evaluates resident needs, supports CNAs, and ensures HIPAA compliance and confidentiality. The position is PRN (As needed, 7am–7pm or 7pm–7am). Responsibilities Perform resident assessments and develop care plans. Implement and evaluate interventions based on resident needs. Provide direction and assign daily duties to nursing staff. Act as a resource for staff concerning resident care and keep management abreast of status. Implement physician orders and administer medications and treatments. Participate and comply with all internal CQI and external regulatory requirements. Document relevant information in resident’s chart, including nursing interventions and any significant changes in mood, behavior, affect or cognition. Use and adapt to new technologies, including system, tools, equipment and the SARA communication system. Collaborate with resident, family and team to formulate individualized care plans. Coordinate care and manage outcomes with the team and management. Implement appropriate care to meet spiritual, physical, psychological and social needs. Act as an advocate for residents, staff and others and treat others with respect. Maintain positive attitude, initiative and the ability to multitask and work independently or as part of a team. Meet required physical capacity: walk, stand, reach, bend, squat frequently, lift 25 pounds without assistance, 26+ pounds with manual or mechanical assistance. Shift Details PRN; As needed (7am–7pm or 7pm–7am). Qualifications and Competencies Strong clinical skills. Ability and knowledge to care for geriatric residents. Assess, implement, evaluate and/or re-evaluate resident care needs. Positive attitude and initiative. Ability to multi-task and work as a team as well as independently. Act as a resource for staff concerning resident care and keeps management abreast of status. Implement physician orders and administer medications and treatments. Participate and comply with all internal CQI and external regulatory requirements. Document pertinent information in resident’s chart such as nursing interventions, any problems noted, limitations or significant change in resident’s mood, behavior, affect or cognition. Meet all documentation standards and follow through with all charting requirements. Use and adapt to new technologies, including tools, equipment, methods of application, computerized system, SARA communication system, etc., plus keep job-required technical skills up to date. Collaborate with resident, family and team to formulate individualized care plan. Work collaboratively with the team and management to coordinate care and manage outcomes. Implement appropriate care to meet spiritual, physical, psychological and social needs. Act as an advocate for residents, staff and others and treat others with respect. Required Education and Experience Valid license as a Practical Nurse in the State of Virginia in good standing. CPR, AED and First Aid Certification.
