Key Responsibilities\n

• Act as a liaison between clients, their families, and outpatient providers to ensure seamless continuity of care.

• Engage with insurance companies for pre-certification and utilization management, advocating diligently for client needs.

• Play an active role in discharge and aftercare planning, ensuring clients have the support they need post-treatment.

• Provide therapeutic meal support, guiding clients to develop positive relationships with food by modeling healthy behaviors.

• Deliver safe, empathetic, and affirming care tailored to each client's unique circumstances and intersecting identities, enhancing their therapeutic experience.

• Conduct assessments and develop individualized treatment plans based on thorough evaluations of client needs.

Patient Population / Client Focus\n

Our primary focus is on individuals seeking support during detoxification and recovery. Clients often present with complex mental health challenges alongside substance use disorders. You will be working with a diverse population, including older adults, addressing their unique emotional and psychological needs in a respectful and understanding manner. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Possession of a valid Licensed Clinical Social Worker (LCSW) license in New York State is essential.

Compensation and Benefits\n
