Location: Austin, TX

Setting: Home Health

Pay Range: 1977.92 - 2076.82

Schedule: Discuss with recruiter.

Contract Length: 13 weeks

Candidate Type: Traveler

Requirements: RN License; BLS; PALS; ACLS; 2 years current experience in Pediatric ICU; at least 1 travel assignment

Responsibilities:

• Assess home health and hospice patients in stable and emergent situations to identify their physical, psychosocial, and environmental needs.
• Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Obtain data on factors influencing patient/family health status and incorporate it into the Plan of Care.
• Maintain accurate and up-to-date patient records reflecting all visits, contacts, problems, plans, actions, and goals.
• Provide care utilizing infection control measures in compliance with OSHA regulations.
• Educate patients and families on care, medications, diet, home safety, and self-care practices.
• Supervise and provide guidance to HHA/STNA/LVN to ensure quality service.
• Assess and reassess pain, utilizing appropriate pain management techniques, and educate patients and families.
• Arrange referrals to appropriate agencies when needed.
• Monitor assigned cases to ensure compliance with third-party payer requirements.
• Perform on-call responsibilities and provide on-call services as assigned.
• Communicate effectively with patients, families, team members, and other healthcare professionals.
• Attend interdisciplinary team meetings and other related meetings as necessary.
• Perform other duties as assigned.
