Clinical Pharmacist/ Left Ventricular Assist Device (LVAD) Anticoagulation Clinic

Overview The highly desired candidate will have experience in managing complex anticoagulation patients (warfarin, DOACs) or managing LVAD patients. This day shift, M-F Clinical Pharmacist role is located at our EUH – Clinic B. Description JOB DESCRIPTION: The clinical pharmacist practices as a pharmacy generalist in one or more patient care areas. The position is responsible for participating in departmental or institutional quality, safety, and cost‑saving initiatives. Responsibilities include ensuring appropriate therapy and institutional outcomes; providing patient‑centered, interdisciplinary pharmaceutical care; proactively reviewing and processing orders accurately and efficiently; preventing and/or resolving drug‑related problems to ensure safety, efficacy, and cost‑efficiency by consulting with ordering prescribers and appropriately documenting; performing daily in‑depth patient profile reviews to assess, monitor, and recommend appropriate drug therapy; evaluating, intervening, and documenting clinical interventions per institutional protocols and guidelines; evaluating the usefulness of medical literature to optimize evidence‑based care; monitoring drug therapy regimens and making recommendations or modifications per protocol; utilizing system nomograms and other guidelines; documenting clinical activities and adverse drug events; attending medical codes as applicable; following, maintaining, and reinforcing system policies and processes for proper dosing, storage, handling, dispensing, and/or administration of medications and compounded products; handling conflict and problems, including ethical issues, constructively and appropriately; engaging with other clinical pharmacists and leadership as needed; participating in coordination of transitions of care; ensuring service completion and patient satisfaction; maintaining clinical competency through licensure and departmental requirements; serving as preceptor for pharmacy students or providing at least one educational in‑service annually; assisting in training of the pharmacy department and other disciplines; participating in research and publication opportunities; actively engaging in departmental mentorship and collaboration programs; recognizing process failures and engaging in analysis and improvement efforts; participating in at least one medication‑use evaluation or quality improvement project annually; overseeing work performed by pharmacy students, technicians, and pharmacists; documenting adverse drug reactions and medication variances per policy; reinforcing medication‑use policies and processes for financial medication stewardship; supporting cost‑savings initiatives; engaging in review of medication insurance authorization status; maintaining understanding of 340B practices; and performing other duties as assigned or requested. Minimum Qualifications One year of relevant experience, Pharmacy Specialty Board certification or ASHP‑Accredited Post Graduate Year 1 pharmacy residency preferred. One year of experience waived for candidates who have been employed as student/interns within the Emory Healthcare system. Eligibility for Georgia Licensure; candidate must be licensed in Georgia prior to official start date. Basic Life Support (BLS) Certification within 90 days, and Advanced Cardiac Life Support (ACLS) within 6 months, if directly applicable to practice specialty. Physical Requirements Medium: lifting 20‑50 lbs (0‑33 % of the work day occasionally); lifting 11‑25 lbs (34‑66 % of the work day frequently); lifting 1‑10 lbs (67‑100 % of the work day constantly); maximum 50 lbs; carrying objects up to 25 lbs; occasional to frequent standing & walking; occasional sitting; close eye work. Environmental Factors Exposure may include blood‑borne pathogens, bio‑hazardous waste, chemicals/gases/fumes/vapors, communicable diseases, electrical shock, floor surfaces, hot/cold temperatures, indoor/outdoor conditions, latex, lighting, patient care/handling injuries, radiation, shift work, travel, and use of personal protective equipment including respirators. Environmental conditions may vary depending on assigned work area and tasks.
