Registered Nurse, Home Care

Job Summary Mass General Brigham Home Care, Inc. is seeking a full‑time Registered Nurse to join the “Constitution” team. The RN will provide high‑quality, autonomous clinical care in patients’ homes across Medford, Everett, and the surrounding areas (Arlington, Malden, Melrose, Somerville). The role incorporates case management duties, coordinating interdisciplinary care teams, and performing 5‑6 visits per day with built‑in time for documentation, chart review, and care coordination. Responsibilities The RN will serve as a case manager, overseeing the full care plan while collaborating closely with LPNs, therapists, and social workers. You will provide skilled nursing care—including assessment, teaching, treatment, documentation, and care coordination—while ensuring compliance with MGB Healthcare at Home policies and standards of practice. Daily activities include: Conducting home visits and delivering clinical services in various community settings. Completing electronic documentation and interdisciplinary communications in a timely manner. Ensuring financial and quality customer service awareness during care delivery. Qualifications Graduate of an approved School of Nursing. Current Massachusetts license as Registered Nurse. Minimum one year of acute‑care nursing experience; prior home‑care experience preferred. Travel required within MGB Healthcare at Home geographic area. Proficiency with various computer software and knowledge of hand‑held patient care computer devices preferred. Strong interpersonal and customer‑service skills; ability to work with a diverse population. Excellent written, verbal, and electronic communication abilities. Strong organizational skills, ability to prioritize work, and adapt to changing situations. Ability to work independently and as part of an interdisciplinary health‑care team. Additional Job Details Mass General Brigham Home Care provides non‑acute, medically necessary skilled care in the home. Services include skilled nursing care (IV drug administration, injections, tube feeding, wound care, care plan education), physical therapy, occupational therapy, speech language pathology, medical social services, and necessary medical supplies.
