Registered Nurse Hospice
Location: South Bend, IN Schedule: Full-Time | MondayFriday | 9:00 AM 5:00 PM Setting: Community-Based Hospice (On-Site/Field Work)
As a Hospice RN, you will:
- Provide compassionate, patient-centered hospice nursing care in homes, facilities, or community settings
- Perform assessments, develop individualized care plans, and monitor patient progress
- Educate families on care needs, comfort measures, and available support
- Collaborate closely with the interdisciplinary hospice team
- Provide emotional support to patients and loved ones with warmth and respect
Candidate Requirements:
- Active RN license (Indiana)
- Minimum 2 years of hospice experience
- Reliable transportation for field visits
