You will support members in achieving sustainable behavior change, navigating health resources, and meeting their wellness goals. As part of an integrated care team, you will collaborate closely with RN care managers, registered dietitians, medical providers, and behavioral health clinicians. This role requires strong clinical boundaries, exceptional communication skills, and the ability to manage a high-volume coaching schedule.Important Requirements (Must Meet to Be Considered)National Board-Certified Health & Wellness Coach (NBC-HWC) – RequiredBilingual in Spanish – Strongly preferred.2+ years of professional health coaching experienceWeight-management experience requiredCertified Diabetes Prevention Program (DPP) Lifestyle Coach – Strongly preferredExperience working with patients using GLP-1 medications – PreferredMust be able to work Pacific Time hours (8am–6pm PT)Must be comfortable completing up to 10 coaching visits per day (phone or video)Must submit a recorded assessment as part of the application processKey Responsibilities1:1 & Group CoachingConduct 30-minute individualized coaching sessions using motivational interviewingCo-create SMART goals and provide ongoing accountabilityFacilitate group coaching sessions and wellness workshopsAdminister screening tools and health measurementsMaintain accurate, timely documentation in the EMRCare Team CollaborationWork closely with RNs, dietitians, medical providers, and behavioral health cliniciansSupport structured weight-management programsProvide behavior-change expertise to the multidisciplinary teamCommunicate via Slack and EMR to coordinate careConduct proactive outreach to re-engage membersOperations & ComplianceManage scheduling, follow-ups, and member panel activityParticipate in wellness campaigns and client eventsAdhere to NBHWC Code of Ethics, HIPAA, PHI/PII protocols, and IT security standardsMaintain a private, professional workspace for virtual visitsParticipate in call audits, QA reviews, and feedback cyclesPreferred Skills & ExperienceExperience delivering care via telehealthSalesforce and Google Workspace proficiencyStrong understanding of coaching vs. education vs. behavioral health boundariesProven track record of patient engagement and retentionWorking ConditionsStationary work for extended periodsDaily use of computer, headset, and video conferencing toolsOccasional travel to client sites for events or meetingsApplication RequirementAll applicants must submit a recorded assessment demonstrating coaching style, communication skills, and bilingual fluency. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
