Physical Therapist (PT) Per Diem | $70 - 85 per Visit 1:1 Homecare

BrightStar Care of Westfield & Summit is seeking an experienced, compassionate Physical Therapist (PT) to provide one-on-one home health care. This is a per-diem role with no minimum hours requiredperfect for therapists who value flexibility and meaningful patient connections.

What We Offer:

What Youll Do:

• Evaluate patients, develop personalized care plans, and provide skilled PT treatments
• Collaborate with physicians & the Director of Nursing
• Teach patients, families, and caregivers safe mobility & exercise techniques
• Monitor progress, document outcomes, and support discharge planning

What We Require:

• Masters degree (or higher) in Physical Therapy from an accredited program
• Active NJ PT license + CPR/BLS certification
• Minimum 1 year of PT experience (home health preferred)
• Commitment to compassionate, high-quality care
