Field-Based Community Health Advocate
IMPaCT Care
IMPaCT Care, embedded with Kaiser Permanente in Atlanta, GA, seeks a Community Health Worker to bridge patients with care in the community. You’ll build trust, navigate social needs, and coordinate with care teams. This field-based role requires a High school diploma, 1–3 years in outreach, a valid driver’s license, and ability to travel across the Atlanta area.
