Gentiva in Maine is seeking an experienced Hospice Clinical Liaison to ensure efficient intake and admissions, while building strong professional relationships with referral sources and the hospice care team. You will coordinate and educate patients and families about hospice services, and support discharge planners and physicians in daily rounds. The role requires RN licensure with hospice experience; BSN is preferred, and you will travel to hospitals and facilities to maintain visibility and #J-18808-Ljbffr
