Bilingual Spanish Community Health Worker

Oak Street Health is hiring a Community Health Worker in Flushing, NY, bilingual Spanish required, to bridge patients, community, and medical systems. You will conduct in-person and phone outreach, promote health literacy, and connect patients to resources while adhering to HIPAA. You will work with Medical Social Workers, manage referrals, and visit patients in clinics, homes, or community sites, spending about half your time outside the center with reliable transportation. 40 hours weekly. #J-18808-Ljbffr Oak Street Health
