Nurse Practitioner

Job Summary: Provides direct care to patients and serves as a consistent care coordinator through the continuum of care, assessing the physical, psychological, spiritual and social needs of the patient. Directs patients to the next appropriate service and tracks their progress. In collaboration with the Breast Center Medical Director, provides health education, promotes wellness and performs a health assessment and physical examination of the breasts.##Working collaboratively with the New York State #Department of Health Cancer Services Program for the Hudson Valley #Region, provides breast health education, screenings, diagnostic, treatment and support services, i.e. case management to medically underserved women, 40 years #of age and older, residing primarily in Rockland County. Develops and implements community outreach programs geared to reduce the glaring disparities in breast cancer#s impact among the area#s traditionally underserved. Essential Job Qualifications ###### 1.### Education: Masters Degree in Nursing, Advanced Nurse Practitioner ###### 2.### Experience: Five years nursing experience, One year Nurse Practitioner preferred. ###### 3.### Training/Certification: Family Nurse Practitioner ###### 4.### Licensure: Registered Nurse ###### 5.### Other: Bilingual in Spanish preferred. # #
