Overview HealthPlus Staffing is seeking a Registered Nurse Case Manager to provide care and support to terminally ill patients. This will be a full-time position primarily serving Merced and South Stanislaus Counties. Responsibilities Provide compassionate hospice care to terminally ill patients, focusing on symptom management and emotional support. Coordinate and deliver patient care as a nurse case manager in the Merced and South Stanislaus Counties region. Apply nursing care processes and adhere to medication guidelines and side-effects management. Maintain high ethical standards and safety precautions while delivering care and support to patients and families. Qualifications Minimum one year nursing experience; hospice or home health experience preferred. Knowledge of nursing care processes and medication guidelines/side-effects. Strong commitment to safety, ethics, and patient confidentiality. Proficient in reading and writing English. Excellent communication skills and a compassionate nature. Cool-tempered, patient, with good emotional stability and stamina. California Registered Nurse license required. CPR certification current. Must have reliable transportation and auto insurance. Hospice experience preferred.
