OB/GYN Case Manager RN — Multidisciplinary Care Lead

KAISER in San Diego seeks a Case Manager Specialist RN (Obstetrics/Gyn) to work with physicians and the multidisciplinary team to manage complex patient needs. You will assess patient needs, develop and implement care plans, coordinate access to services, monitor progress, and educate patients for self-management and empowerment. Requires California RN license and BLS, with 2+ years of clinical RN experience; Bachelor’s degree in nursing or four years of equivalent experience is expected. #J-18808-Ljbffr Kaiser Permanente
