Key Responsibilities

• Conduct comprehensive evaluations of residents' physical capabilities, identifying their strengths and areas for improvement to inform targeted treatment plans.
• Design and implement individualized rehabilitation programs that focus on enhancing each resident's strength, mobility, and functional independence through tailored therapeutic exercises and manual therapy techniques.
• Utilize a variety of therapeutic modalities, including electrical stimulation and ultrasound, alongside hands-on care to address physical impairments effectively.
• Regularly monitor and assess residents' progress, documenting outcomes and adjusting treatment plans in collaboration with the healthcare team as necessary.
• Engage with an interdisciplinary team, including physicians, nurses, and occupational therapists, to create a cohesive care strategy that addresses all aspects of the residents' health and well-being.
• Provide education to residents and their families on home exercise regimens and lifestyle strategies that promote ongoing physical health and prevent further impairments.
• Maintain compliance with state and federal regulations and industry standards to ensure quality and safe delivery of care.

The ideal candidate will work with a diverse population, primarily consisting of elderly residents and individuals recovering from surgeries or acute injuries. This demographic requires a compassionate approach and a nuanced understanding of the unique challenges faced by older adults or those with complex medical histories. You will play a pivotal role in guiding residents through their rehabilitation journey, restoring their functionality and independence.

Qualifications

• Current licensure as a Physical Therapist in the state of New York.
• A Bachelor's Degree in Physical Therapy from an accredited institution; a Master's Degree or Doctorate in Physical Therapy is preferred.
• Strong communication, organizational, and interpersonal skills, with the ability to engage effectively with residents and their families.
• Proficient in a range of therapeutic modalities and techniques relevant to physical rehabilitation and therapy.

Preferred Experience

• A minimum of 2 years of experience as a Physical Therapist is required; experience in skilled nursing facilities or working with the geriatric population is highly desirable.

• Adept at working collaboratively within an interdisciplinary team, demonstrating leadership and teamwork skills.

Compensation and Benefits
