Physical Therapist (PT) – Home Health Case Manager Location: Chicago North Side (Irving Park, Albany Park, Uptown, Lakeview, and surrounding areas) Schedule: Day shift with weekend and holiday rotation Sign-On Bonus: Eligible for up to a $10,000 sign-on bonus (external candidates only). Must be employed in the same role at time of payout. As part of a dynamic clinical team within Advocate’s integrated health care organization, home health offers the therapist the opportunity to work independently to assess and address the mobility, safety, communication, and equipment needs of patients. As case managers, therapists establish strong relationships with patients and families and consult with physicians and other health care providers as necessary. What You Need Licensed Physical Therapist in the State of Illinois (or eligible for licensure). Completed 1 year of Physical Therapy clinical experience or demonstration of a strong spectrum of Physical Therapy clinical affiliation experience. Proficient use of home-based therapy equipment and assistive devices (balls, bands, weights, Hoyer lifts, etc.). Basic knowledge of computer use (Microsoft Word, email). Ability to learn to use computer-based scheduling and documentation system. Ability to communicate professionally both verbally and through written reports, with strong documentation skills. Good interpersonal skills, time management, and self-organization skills. What You Will Do Travel extensively within geographical boundaries in all weather extremes. Work rotating weekends and holidays. Adhere to scheduled days and hours of work based on commitment to patients/clients and workload assigned. Attend mandatory informational, education, and branch meetings. Requires full range of body motion, including bending, sitting, standing, climbing stairs while carrying objects, pulling, pushing, reaching, carrying and lifting (must lift/carry a minimum 25 pounds). Assist and maintain patient safety with ambulation on flat surfaces and stairs and transferring patients using standard pivot transfers or transfer board/Hoyer lift. Provide treatment on standard beds in the home environment. Lift up to 35 pounds without assistance. For patient lifts over 35 pounds, use patient-handling equipment with at least one other associate when available. Unique patient lifting/movement situations will be assessed case-by-case. Assess, plan, implement, and evaluate physical therapy treatment programs established for the home-care patient. Facilitate the progression of the patient in their home toward optimal levels of self-care. Accountabilities Assure and improve quality of care: facilitate the patient and family's right to receive quality cost-effective care following Advocate's MVP of Stewardship. Care and service coordination: practice as an effective member of the patient care team to formulate an integrated approach to care following Advocate's MVP of Partnership. Leadership, collaboration, and professional development: demonstrate respect and understanding for peers and other clinical disciplines and participate as an effective member of the patient care team to formulate an integrated approach to care following the Advocate MVP of Excellence. Patient relationship: develop and maintain a restorative relationship throughout the healthcare continuum, following Advocate's MVP of Compassion. Business development and growth: demonstrate behaviors that enhance productivity and an understanding of financial and economic conditions in the health care environment. Patient education: demonstrate a commitment to meet the learning needs of patients and their families. Compensation $86,320.00-$129,376.00 annual base salary, incentive eligible.
