Responsibilities

• Provide physical therapy assistant services under the supervision of a licensed PT, contributing to treatment plans for geriatric patients in an LTAC setting.
• Implement and document therapeutic interventions, gait training, transfers, balance activities, ROM, strength, and functional mobility programs.
• Monitor patient progress, adjust activities within scope of practice, and communicate changes to the supervising PT and care team.
• Educate patients and families on home exercise programs, safety, fall prevention, and activity modification to support discharge planning.
• Maintain accurate and timely documentation of sessions, outcomes, and any notable changes in patient status.
• Ensure infection control, safety standards, and ethical practice during all rehab activities.

Requirements

• Active Physical Therapy Assistant license in New Hampshire.
• Associate degree from an accredited Physical Therapist Assistant program.
• CPR/BLS certification (preferred).
• Prior PT aide/assistant experience in LTAC or skilled nursing facility settings is preferred.
• Ability to perform duties in a fast-paced LTAC environment and collaborate with a multidisciplinary team.
