Our organization’s Patient-Centered Medical Home Primary Care Clinics represent Army Medical Homes that utilize an interdisciplinary approach to deliver comprehensive, evidence-based primary care. This model emphasizes the coordination of care beyond the primary setting and actively involves patients as partners in their health journey.

The core principle of the Army Medical Home is to prioritize patients. It serves as a gateway for Army Medicine to influence key health determinants such as sleep, activity, and nutrition, collectively known as the Performance Triad.

Each patient collaborates with a dedicated team of healthcare professionals—including physicians, nurses, behavioral health specialists, clinical pharmacists, physical therapists, and case managers—to create a personalized health care plan. This consistent team structure enhances continuity of care and fosters a stronger partnership between patients and their healthcare providers. The Army Medical Home model has demonstrated improvements in overall patient health, leading to reduced emergency visits, fewer hospital admissions, shorter inpatient stays, and lower readmission rates.

The Puyallup Community Medical Home encompasses facilities in the Puyallup area, primarily the Madigan-Puyallup Community Medical Home, which is part of the Madigan Army Medical Center, and the Puyallup Medical Clinic (Community Health Care). The former focuses on military families (TRICARE), while the latter addresses broader community health needs.

Key Responsibilities

• Deliver comprehensive primary care utilizing independent clinical judgment, including diagnosis, treatment, prevention, and referrals across various medical conditions.
• Manage a designated patient panel, ensuring coordinated and continuous patient-centered care across multidisciplinary teams and the healthcare system.
• Accurately document all clinical services in compliance with regulations, policies, and established procedures.
• Provide teaching and consultation to other healthcare providers, and engage in Continuing Medical Education, quality assurance, and performance improvement initiatives.
• Supervise assigned healthcare staff, including task assignment, technical oversight, performance evaluation, candidate interviews, personnel recommendations, and addressing training and disciplinary matters.
• Perform additional duties as assigned.

Qualifications

Eligibility: Open to U.S. Citizens

To qualify for this position, candidates must meet the following educational and experiential criteria:

• Basic Requirement: A Doctor of Medicine or Doctor of Osteopathic Medicine degree from an accredited institution in the U.S. or Canada. This degree must be recognized by relevant accrediting bodies. For foreign medical degrees, equivalency must be established through certification by the Educational Commission for Foreign Medical Graduates or similar credentials.
• Licensure: A current, unrestricted license to practice as a Physician in a U.S. state, the District of Columbia, Puerto Rico, or a U.S. territory is required.
• Graduate Training: At least one year of supervised clinical experience post-degree, such as an internship or the first year of a residency program in an accredited facility.
• Residency Training: Successful completion of an accredited residency in Family Practice or Family Medicine, along with equivalent experience totaling at least four years. Experience cannot substitute for the required residency training.
