Job DescriptionJob DescriptionBenefits:401(k)Company parties Licensed Vocational Nurse (LVN) – Congregate Living Health Facility (CLHF) Job Summary We are seeking a compassionate, dependable, and clinically skilled Licensed Vocational Nurse (LVN) to provide direct nursing care to medically fragile residents in a Congregate Living Health Facility (CLHF). Under the supervision of a Registered Nurse (RN) or physician, the LVN is responsible for delivering safe, high-quality nursing care in accordance with physician orders, individualized care plans, facility policies, and applicable California regulations. Essential Duties & Responsibilities Provide direct nursing care according to physician orders, resident care plans, and facility policies and procedures.Administer medications, treatments, enteral feedings, and other prescribed therapies.Perform skilled nursing procedures within the LVN scope of practice, including wound care, catheter care, tracheostomy care, and suctioning.Monitor residents’ physical, emotional, and clinical status and promptly report significant changes to the RN, physician, and/or Director of Nursing (DON).Accurately and timely document assessments, nursing interventions, treatments, medication administration, and resident responses.Maintain accurate Medication Administration Records (MARs) and Treatment Administration Records (TARs).Assist with resident admissions, transfers, discharges, and physician appointments.Communicate professionally and effectively with physicians, pharmacists, families, and members of the interdisciplinary care team.Recognize and respond appropriately to emergencies and changes in resident condition.Supervise and provide direction to Certified Nursing Assistants (CNAs) and other assigned staff.Follow infection prevention and control policies and procedures.Participate in resident care planning and interdisciplinary team meetings.Maintain resident confidentiality, dignity, privacy, and rights in accordance with HIPAA requirements and facility policies.Support compliance with applicable California Title 22 regulations and CLHF licensing requirements.Participate in quality assurance and performance improvement activities.CLHF-Specific Responsibilities Provide nursing care to residents with complex and medically fragile conditions, including residents who may require: Enteral feeding through G-tube/PEG tubeCare related to neurological conditions and spinal cord injuriesRespiratory treatments and oxygen therapyThe LVN will closely monitor residents’ respiratory and overall clinical status, immediately report significant changes, and assist with emergency response and resident stabilization until a higher level of medical assistance is available. Required Qualifications Current and active California LVN license in good standingCurrent CPR/BLS certificationKnowledge of medication administration and skilled nursing proceduresAbility to work independently while following appropriate clinical supervision and facility protocolsAbility to recognize changes in resident condition and exercise sound clinical judgment within the LVN scope of practiceStrong communication, organization, and documentation skillsAbility to work effectively as part of an interdisciplinary healthcare teamPhysical Requirements Candidates must be able to: Stand, walk, bend, and reach throughout the shiftAssist with lifting, repositioning, and transferring residents using appropriate assistance and equipmentRespond promptly and safely during emergency situationsSchedule & Compensation Shift Length:10-12-hour shiftsCompensation: Competitive pay based on experienceJoin Our Team If you are a compassionate and reliable LVN who enjoys providing hands-on care to medically complex residents, we encourage you to apply.
