Physical Therapist (PT)

The Physical Therapist (PT) Home Health plays a vital role in restoring mobility and function for patients receiving care in their homes. This position requires strong clinical skills, problem-solving abilities, and a patient-first mindset. The PT will provide personalized therapy plans, educate patients and caregivers, and ensure compliance with state and federal healthcare regulations. This is a field-based role requiring travel within Eugene and surrounding areas (including Salem when needed). Expertise in physical therapy techniques, rehabilitation exercises, and functional training. Ability to assess and modify treatment plans based on patient progress. Strong problem-solving and clinical decision-making skills. Proficiency in documenting patient care and maintaining compliance records. Education, Certification & Experience Education: Doctorate or Master's (MS) in Physical Therapy required. Licensure: Active Physical Therapist License in the state of Oregon. Certifications: Current CPR/BLS certification required. Experience Minimum one year of experience as a Physical Therapist. Home health experience preferred. Other Requirements Valid drivers license with reliable, insured transportation. Willingness to travel within Eugene, Salem, and surrounding areas as needed. #J-18808-Ljbffr
