Minimum Qualifications Associate degree in Physical Therapy from an accredited program. Active Illinois PTA license required to start the assignment. BLS certification from AHA or ARC preferred. Experience working in a skilled nursing facility required. Experience with 5x8 day schedules and documentation in long-term care preferred. Ability to collaborate with interdisciplinary teams and provide restorative care.

Assignment Details Facility Type: Skilled Nursing Facility. Shift: Days. Shift Hours: 08:30–16:30. Work Pattern: 5x8. Start: ASAP. Assignment Length: 14 weeks.
