Palliative Care Nurse Practitioner Job-4 Day Week on Whidbey Island, WA

A leading healthcare organization on Whidby Island in Northwest Washington is seeking a compassionate and dedicated Palliative Care Nurse Practitioner to join its interdisciplinary care team. This role supports patients with life?limiting, progressive, and terminal illnesses, with a focus on alleviating suffering, improving quality of life, and guiding patients and families through complex care decisions.

This role includes care across multiple settings, including home?based visits, outpatient consultations in primary care and oncology, and inpatient support for transitions of care and hospice evaluations.

Compensation & Benefits:

• Salary Range: $130,000 – $140,000 annually

• Potential sign?on incentive

• Employees at 0.6 FTE or higher qualify for full benefits, including health, dental, vision, retirement options, and paid leave

Schedule & Work Hours:

• This is a full?time position structured as:

• Four 10?hour shifts per week

• Day shift coverage

• Consistent schedule that supports work?life balance while providing continuity of care for patients and families

Key Responsibilities:

• Conduct comprehensive and problem?focused physical, psychosocial, cultural, and spiritual assessments

• Order and interpret diagnostic tests aligned with palliative goals of care

• Develop differential diagnoses and identify expected outcomes

• Create and implement individualized, evidence?based, culturally sensitive care plans

• Prescribe and manage medications to address pain and complex symptoms

• Integrate nonpharmacologic and complementary therapies into care plans

• Provide counseling to patients and families across the disease trajectory, including bereavement support

• Ensure accurate documentation to support continuity of care and appropriate reimbursement

• Participate in quality improvement initiatives and interdisciplinary collaboration

• Contribute to formal and informal palliative care education within the organization and community

• Demonstrate knowledge of hospice benefits, transitions of care, and collaborative practice with hospice medical directors

Qualifications:

Master of Nursing or Doctorate of Nursing Practice from an accredited program

Graduate?level training in palliative care or a minimum of 3 years of palliative or hospice care experience

National board certification as an Adult or Family Nurse Practitioner

ACHPN certification preferred, or willingness to obtain within 12-18 months
