Pay Range $38.20 - $57.30 Location This position is for a Medicare Wellness Nurse who will support multiple primary care clinics in the Concord area. The Medicare Wellness Visit (MWV) focuses on preventive care, helping patients close care gaps and meet quality measures while maintaining strong engagement with their primary care team. In this role, the RN will obtain vital signs, administer Medicare-covered vaccines, complete a comprehensive health risk assessment, and develop a personalized prevention plan for each patient. Responsibilities Perform Medicare Wellness Visits (MWV) for patients, including vital signs, vaccine administration, health risk assessment, and personalized prevention plans. Work 8 a.m. - 5 p.m., Monday-Friday. Use Epic electronic health record system effectively. Work independently while demonstrating compassion and a commitment to continuous improvement. Qualifications Completion of an accredited or approved nursing program. Registered Nurse license issued by the state in which the teammate practices. Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association. Adult care nursing experience of any length. Minimum 5 years of nursing experience (excluding schooling/practicum). Bachelor of Science in Nursing (BSN) preferred. Board certification in Gerontology or Ambulatory Nursing required within 2 years of hire. Benefits Competitive base salary within pay range.
