Nurse Navigator RNJamestown Office - Jamestown, ND 58401OverviewSalary Range $34.67 - $52.01 Position Type Full Time Job Shift DayDescriptionOur mission: 'To provide comfort and care through life's journey' Join our team today!General Summary:As a Hospice Nurse Navigator, you will serve as a trusted clinical advisor and community liaison, working across healthcare settings to identify patients appropriate for hospice care, educate providers and partners on hospice services and benefits, coordinate seamless transitions into care, and build strong, lasting referral relationships. This role combines clinical navigation + outreach strategy, making it ideal for an RN who enjoys both care coordination and community engagement.This position is full-time and based out of our Jamestown office location and serving the surrounding communities.Essential Functions:Guide providers and families in understanding hospice eligibility and care optionsFacilitate hospital referrals, admissions, and care transitionsCoordinate with clinical and admissions teams to ensure timely patient accessSupport documentation and assist with physician orders for admissionProvide in-services, presentations, and one-on-one education to healthcare partnersPromote understanding of hospice philosophy, benefits, and servicesProvides education and training to internal team members and partner facility staff to promote collaboration, strengthen clinical best practices, and ensure consistent, high-quality care delivery across the continuum.Serve as a subject matter expert in end-of-life careDevelop and maintain strong referral partnerships with physicians, hospitals, and community organizationsExecute a strategic territory plan to grow hospice awareness and appropriate referralsParticipate in understanding referral trends and adjust outreach effortsServe as a visible, trusted presence throughout the healthcare communityCollaborate with community relations, marketing, education, and clinical teamsBenefits:We offer a robust benefits package for qualifying full-time and part-time employees.
