Weekend Admissions nurse

Job Summary Registered nurse plans, organizes, and directs/coordinates home care services with emphasis on community health education. The professional nurse utilizes community resources to plan and direct services to meet the needs of individuals and families within their homes and communities. Duties & Responsibilities Develop and initiate the plan of care and assume responsibility for ongoing interdisciplinary assessment and development of individualized plans in partnership with patients, their representatives, and caregivers. Initiate proper preventive and rehabilitative nursing procedures and provide services ordered by the physician as indicated in the plan of care. Provide patient, caregiver, and family counseling. Provide patient and caregiver education. Prepare clinical notes. Communicate with the physician responsible for the home health plan of care and other healthcare practitioners related to the current home health plan. Identify discharge planning needs as part of the care plan development and implement prior to patient discharge. Communication Prepare clinical notes and update the primary physician at least every sixty days. Communicate with the physician regarding patient needs and report any changes in the patient’s condition; obtain/receive physician’s orders as required. Communicate with community health personnel to coordinate the care plan. Additional Duties Participate in the HHA's quality assessment and performance improvement program and HHA-sponsored in-service training. Participate in on-call duties as defined by the on-call policy. Ensure that arrangements for equipment and other necessary items and services are available. Instruct, supervise, and evaluate home health aide care provided every two weeks. Supervise Licensed Practical Nurses, Licensed Vocational Nurses, and/or Home Health Aides/Certified Nursing Assistants as directed. Job Requirements Graduate from an approved school of nursing and be licensed in the state where practicing. Minimum of two years of experience, with at least one year in public health or home care nursing preferred. Maintain a current CPR certification. Be a licensed driver with an automobile that is insured and in good working order as required by state or organization. Self‑directed and able to work with minimal supervision. Demonstrate excellent observation, problem‑solving, verbal and written communication skills and nursing competency. Show ability to organize and prioritize workload independently. Management experience not required; responsible for supervising home health aides. Able to perform prolonged walking or standing; lift, position, transfer patients and supplies; perform reaching, stooping, bending, kneeling, or crouching; have adequate visual acuity and hearing. Qualifications Registered Nurse (RN) CPR certification 1 year Home Health experience preferred Knowledge of Home Care Home Base beneficial Additional Information COVID‑19 VACCINE REQUIRED.
