Licensed Practical Nurse

LPN PositionSchedule: Full-Time (7am-3pm) Location: Queens and BrooklynThe LPN will coordinate all medical/psychiatric care and medications for residents ensuring proper documentation, follow-up and regulatory compliance. Assist with hands-on training of residential staff to ensure this compliance is ongoing and current. Secondary responsibilities include resident care and oversight in AOL's.Identify and organize appointments for week in conjunction with RN and ManagerTransports and accompanies residents on as scheduled medical appointments and completes all necessary follow-up upon return from appointmentPerforms delegated nursing tasks as directed by the RN and under the supervision of the RNOversee coordination of all medication administration including verifying prescriptions, ordering medications, regulatory documentation, and staff administration of medications in conjunction with RN and under supervision of RNMaintain medication inventory including prescription medications, over the counter medication and required first aid supplies are in stock at all times and that check expiration dates are routinely checkedEnsures maintenance of all medically related documentation (MAR, MD Consults, etc.)Updates RN daily on resident's medical statusResponsible to complete all documentation prior to end of shift and report all issues/concerns to RN and Residence ManagerCompletes the documentation of Occurrence Reports as requiredReviews and makes daily entries into House Log Book on all pertinent resident/residence related issuesMonitors resident progress and notifies applicable others of concerns, changes in behavior or of progressResponsible to implement all directives issued by RN and Manager as neededHandles telephone calls and inquiries; provides necessary follow-up; keeps family and management notified when necessaryKnows and applies emergency procedures in event of a crisis including fire safetyActs as liaison between Residence, Day Program, Physicians and families on all resident medical related issuesProvides individual residents instruction and/or assistance with daily living skills and rehabilitation or social skills in accordance with Individual Service PlanProvides ongoing training to residents and staff in areas of need identified in Individual Service Plan in conjunction with RN & ManagerProvides hands-on client specific training to staff members as directed by RNAttends annual and semi-annual reviews providing medical input in absence of RN involvementVisits residents in hospital, as needed and coordinate Discharge Plan in conjunction with RNCompletes necessary residential habilitation notes, progress notes and other required documentationComplies with OSHA regulations and universal precautions, AMAP Training, annual recertification training and annual PPD clearance as per agency policyAttends various meetings/in-services, participates in staff meetings in conjunction with mandatory annual in-services as directed by Residence Manager I RNSupervises residents' intake of medication as prescribed in accordance with OMRDD approved Medication Administration ProcedureMaintains friendly and professional working atmosphereMaintains professional appearanceEnsures residents are always treated with dignityBenefits Overview: We offer attractive compensation with comprehensive benefits including: Medical/Dental/Prescription/Vision/Life Insurance;403(b); Credit Union; FSAs; Short-and-Long-Term Disability; Transportation Plan; Generous Paid Vacations and HolidaysQualifications:Education: New York State License NurseExperience: Specialized training in I/DD and/or one year experience in community health care services, excellent verbal and written communication skills, including strong organizational, detail and interpersonal skills; strong team building skills; computer skills and knowledge of word processing; demonstrated ability to work effectively with men and women of diverse races, ethnicities, ages and sexual orientations in a multicultural environment.Special Skills:CPR and First Aid TrainedMust be a team player in order to interact with Administrative and Managerial staff as well as Supervisory and Line Staff, as appropriateHave a strong sensitivity to cultural differences in order to work well with staff, individuals and families served.Able to set limits, maintain role, work collaboratively and respectfullyKeywords: Direct service professional, counselor, residential counselor, direct care counselor, habilitation specialist, human services, social services, social worker, case manager, health services, developmental disabilities, mental health behavioral health, OPWDD, OMRDDS:US IS AN EQUAL OPPORTUNITY EMPLOYERJoin a team of employees who care about the wellbeing of others.
