Field-Based Intensive Case Manager

City and County of Denver is seeking an Intensive Field Case Manager for the Roads to Recovery program. This field-based role will coordinate outreach, in-reach, and wrap-around care for individuals with complex needs across the city. You will work with multiple city agencies and community partners to ensure engagement, referrals, and ongoing support, with emphasis on trauma-informed care and risk assessment.
