Deliver personalized, hands-on therapy care to clients in their homes. Conduct comprehensive assessments and design tailored treatment plans to improve clients' quality of life. Educate clients and caregivers on disease management, adaptive techniques, and home safety. Collaborate with physicians, nurses, and multidisciplinary teams to ensure holistic care. Re-evaluate and adjust care plans to meet evolving client needs.

Licensed Physical therapist (state licensure required). Experience: 1+ years in home health, outpatient, or rehabilitation settings. Strong clinical judgment and ability to work autonomously. Tech-savvy with EHR/EMR systems (e.g., experience with [specific software]). Compassionate communicator with a knack for building trust with clients and families. Flexibility: Willingness to travel locally to serve clients in the following areas (mileage reimbursement provided).

• Area 1: Boston, South Boston, South End, Dorchester, Roxbury, W. Roxbury, Mattapan, Hyde Park, Jamaica Plain, Roslindale.
• Area 2: Quincy, Weymouth, Randolph, Braintree, Brockton.
• Area 3: Cambridge, Allston, Brighton, Brookline, Somerville, Charlestown.
• Area 4: Lynn, Chelsea, Revere, Everett, Malden, Medford, East Boston, Winthrop, Melrose.
• Area 5: Andover, N. Andover, Billerica, Burlington, Woburn, Wilmington, Chelmsford, Dracut, Haverhill, Lawrence, Lowell.
