Case Manager - The Roz

Job Purpose The Case Manager will provide comprehensive case management to clients and their families who are homeless or at risk of homelessness with the goal of increasing their quality of life and maintaining the highest possible ability to function within the community. Essential Job Responsibilities Advocate for and facilitate clients’ access to community resources; housing assistance, utility assistance, and relevant community services and benefits. Build a database of community resources and provide referrals to community resources as needed. Conduct initial intake meetings with the client(s); evaluate individual and family needs; complete service plans with the client, including housing and related needs; coordinate and monitor services, including comprehensive tracking of client activities in relation to service plans and housing inspections; document detailed case notes daily, maintain comprehensive client files; prepare reports as requested by Lead Case Manager, Program Manager, and/or Quality Assurance. Meet regularly with Lead Case Manager to staff case load; provide ongoing program evaluation and recommendations to the Lead Case Manager and Program Director for continuous growth and quality. Be active in and network at monthly community groups, events, and meetings; provide presentations at various community groups, events, and meetings within areas of responsibility, including rural and metropolitan areas; promote the success and reputation of the Emergency Solutions program. Other duties as assigned. Demonstrate exceptional customer service in everything you do by placing the child, family, veteran, or client first to support our mission to “Empower people to build better lives for themselves, their families, and their communities.” Essential Qualifications Education: Bachelor’s Degree in Social Work, Sociology, Psychology, or related field, preferred but not required. Experience: 1+ years of case management experience (3+ years preferred); 2+ years in a customer service focused environment; experience with homeless, low‑income, veterans and their families is a strong plus. Attendance: Must maintain regular and acceptable attendance, including on‑site presence in the office five (5) days per week, as determined by the employer. Licenses: Driver’s License with a clear record. Vehicle: Must have daily access to a vehicle with no prior notice required. Other: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
