Liberty Healthcare - - Responsibilities: Provide patient care services along with direct treatment and follow-up as assigned by the Physical Therapist; Update the care plan in coordination with the therapist and the physician, and plan with the patient/family toward adjustment; Plan monthly patient re-evaluations with the therapist, coordinating with the therapist more frequently if needed; Plan the weekly patient schedule; Document patient care data accurately, thoroughly, and in a timely matter
