Social Services Assistant Vivo Healthcare Taylor - Jacksonville, FL Join our dynamic team at Vivo Healthcare Taylor as a Social Services Assistant! As a critical member of our organization, you will play a vital role in providing exceptional care and support to our patients and their families. About the Role: As a Social Services Assistant at Vivo Healthcare Taylor, you will be responsible for providing social services to patients and their families to ensure a comprehensive approach to their care. You will work closely with multidisciplinary teams to assess and address the social and emotional needs of patients, facilitate referrals to community resources, and provide education and support to patients and their families. Responsibilities: Conduct social work assessments to identify the social and emotional needs of patients and their families Develop and implement individualized care plans to address identified needs Facilitate referrals to community resources, such as support groups, counseling services, and home health care Provide education and support to patients and their families to enhance their understanding of their care plan and promote self-advocacy Collaborate with the care team to address concerns and provide updates on patient progress Document all interactions and care plans in accordance with company policies and regulations Requirements: 0 years of experience in social work or a related field High school diploma or equivalent required; Bachelor's degree in social work or a related field preferred Ability to work in a fast-paced environment with diverse patient populations Strong communication and interpersonal skills Ability to work effectively as part of a multidisciplinary team Familiarity with community resources and ability to navigate systems for patients and families Work Environment: Part-time position (number of hours to be determined) Monday-Friday, 8:00am-4:30pm (flexible schedule may be available) What We Offer: If you are a compassionate and organized individual with a passion for social services, we encourage you to apply to this part-time Social Services Assistant role at Vivo Healthcare Taylor!
