You will utilize your leadership skills in coordinating care and provide home-based nursing care for patients as directed by an attending physician. You will case manage and provide clinical care under the direction of the physician. Youll educate patients and their family members and ensures the safety of the patient. You will work autonomously, so you need to be efficient in managing your time. Youll be responsible for all practices and duties within the scope of practice outlined by the state./ppMajor Areas of Responsibility:/ppPlan of Care: Observe and monitor patient conditions and perform OASIS assessments (If you dont already know this assessment, we will teach you!) to develop an individualized care plan and adjust as needs change./ppMinimize Patient Risk: Administer medication as prescribed by the physician and help decrease re-hospitalizations by prioritizing visits for high-risk patients./ppSupervision: Oversee and supervise total care of patient provided by nurse aides and LPNs/ppContinuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges./ppFamily Educator/Advocate: Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding./ppPolicies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborate with the interdisciplinary team and physicians to ensure optimal care is provided./ppRules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors./ppOperations: Attend in-service training and mandatory agency meetings/ppHard Soft Skills:/ppCompassionate communicator with a positive attitude/ppPatience is a virtue when working with patients, families, physicians, and coworkers/ppAttention to detail is critical, as is being observant and following directions/ppRequirements:/ppRegistered Nurse with current license in the state of employment/ppMinimum one-year experience as an RN in an acute care setting./ppHome care experience preferred/ppValid drivers license and auto insurance in your name as a driver/ppCapable of all physical demands/ppWe are proud to be part of the Alternate Solutions Health Network family./pp#INDRED1/ppWell help you put your passion for patient care to work.
