About the Role:

Responsibilities:

• Administer medications and treatments as prescribed by physicians.
• Monitor patient vital signs and report any changes to the healthcare team.
• Assist with daily living activities to enhance patient comfort and well-being.
• Educate patients and families about health management and wellness.
• Maintain accurate patient records and documentation in compliance with regulations.
• Collaborate with nurses and physicians to develop and implement patient care plans.
• Participate in patient assessments and contribute to care strategies.
• Ensure a safe and clean environment for patients and staff.

Requirements:

• Valid LPN license in the state of Illinois.
• Minimum of 1 year of experience in a healthcare setting preferred.
• Strong communication and interpersonal skills for effective patient interaction.
• Ability to work collaboratively in a team-oriented environment.
• Basic Life Support (BLS) certification required.
• Compassionate demeanor with a commitment to patient-centered care.
• Proficient in electronic health records (EHR) systems.
• Willingness to work flexible hours, including weekends and holidays.

Compensation: $34.00 - $38.00 per hour
