Home-Based LTSS Care Manager
Molina Healthcare of Illinois
Molina Healthcare is seeking a TX licensed LVN, LBSW or LMSW to join our LTSS team as a Case Manager serving Medicaid members in the southern Dallas County area. The role requires face-to-face home visits, comprehensive assessments, and Waiver service planning; travel 25-40% may be required. Strong MS Office skills and coordination with multidisciplinary teams are essential.
