Support providers and registered nurses with patient assessments, treatments, procedures, and ongoing care needs. Administer medications and treatments within the LPN scope of practice and document care accurately in the electronic health record. Monitor patient conditions, identify changes, and communicate effectively with the care team to ensure the best possible outcomes. Educate patients and families on treatment plans, medications, and follow-up care to promote health and healing. Collaborate with physicians, nurses, and other healthcare professionals to create a seamless, patient-centered experience. Help maintain a safe, efficient, and welcoming environment for patients, families, and team members. Champion quality, safety, and continuous improvement initiatives that enhance patient care and outcomes. Maintain current licensure, certifications, and professional competencies while pursuing ongoing learning and development.
