Transitional Facilitator

$26.75 - $32.1 per hour

The Facilitator embraces CII’s North Star and serves children, youth, and families through a whole‑family approach, providing program participants with access to comprehensive services and support to reach their full potential. Principal responsibilities include direct services such as case planning, advocacy, outreach, linkage, and home visitation activities for children, youth, and families in collaboration with a team. The Facilitator uses independent judgment and discretion to conduct regular home visits, assess participant needs, develop meaningful goals, monitor progress, and provide community referrals. They coordinate with educational and employment services, pediatric and dental care, and other supportive services, maintain audit‑ready documentation, participate in crisis intervention, and represent CII at DCFS, DMH, and community meetings. Additional duties may include conducting intake interviews, completing required paperwork, attending educational workshops, reviewing professional publications, and participating in in‑service programs. The Facilitator also maintains knowledge of service standards and policies as stipulated by contract, licensing, and governing bodies, and may undertake special projects as assigned. There are no supervisory responsibilities for this role. Qualifications PFF Home Visitor – Master’s Degree in Social Work, Psychology, Marriage and Family Counseling, or a closely related field; case management experience preferred; early childhood development experience preferred; at least two years of experience in home visitation, child protective services, or therapeutic services; or bachelor’s degree with 4 years of case management experience. Education waivers may be requested from DCFS. FP IHOC – Master’s Degree in a human services field; case management experience preferred; experience with children, youth, and families in the child welfare system preferred. Launch Transition Facilitator – Bachelor’s degree and 4 years of experience providing case management services, or Master’s degree and 1 year of experience. Experience with children, youth, and families in the child welfare system preferred. Other qualifications: Valid driver’s license and state‑required auto insurance; required to travel to sites extensively (over 50% of the time); Spanish/English bilingual preferred; strong ability to work collaboratively, maintain confidentiality and protect PHI, sensitivity to cultural and socioeconomic characteristics, and excellent verbal and written communication skills. Certificates or licenses: None required. Computer Skills Working knowledge of computer programs in a Windows environment, including Word, Excel, and E‑mail. Vision Requirements No special vision requirements. Training New Hire Orientation Mandated Reporter AB-1343: Sexual Harassment Prevention training WRAP program – Wraparound basic training Family Preservation program – Family Preservation basic training Compensation USD 26.75 – 32.10 per hour (in accordance with California law; salary determined based on factors permitted by law).
